Records Request

To Request your Counseling & Psychological Services Record(s): 

• Please complete and sign an Authorization For Disclosure of Protected Health Information Release form. Even if an outside provider has their own form for records release, we require that you complete the UNCG Authorization for Disclosure of Protected Health Information. 
• Fax, mail or hand deliver the Authorization For Disclosure of Protected Health Information Release form along with a copy of your photo ID/student ID (see contact information below)  
• Requests that are not completely filled out, dated, signed and accompanied by a photo ID cannot be processed and will be returned. 
• We cannot accept release authorization forms from outside providers. 
• Please note we cannot accept the Authorization For Disclosure of Protected Health Information Release form. via email. 
• Please allow 7-10 business days to process records requests.
