Immunizations
INCOMING STUDENTS
North Carolina Law requires individuals attending a college or University, public, private or religious to receive certain immunizations.
By now, we hope you are aware of the required immunizations requested by the university. The immmunizations can be entered on Patient Portal. Immunization records can be uploaded on Patient Portal.
Our goal is to receive this information PRIOR to your arrival on campus. Documentation of the immunizations needs to be clear and complete. Please be certain to review the details regarding each requirement.
MEDICAL HISTORY FORM [Required]
The Student Medical Form is designed to collect information about your health history and current immunization status. Please complete and return this form BEFORE you arrive on campus. You should make and keep a copy of your Student Medical Form for future reference.
INTERNATIONAL STUDENTS
INTERNATIONAL STUDENTS and/or non-US Citizens please have vaccination records translated to English prior to submission.
IMMUNIZATION RECORDS MUST INCLUDE
- ● Your name
- ● UNCG Student ID number
- ● Your date of birth (month/year format)
- ● Name, address and phone number of the doctor’s office providing the record
- ● Month/day/year of the immunization received
- ● Make sure the form is completed by a physician or physician’s designee with the signature and/or office signature.
IMMUNIZATION REQUIREMENTS MAY VARY BY AGE
REQUIRED IMMUNIZATIONS
- – DTap/DTP/Td – 3 doses
- – Tdap Booster
- – Polio – 3 doses (only if 17 or younger)
- – MMR ( Measles Mumps Rubella )-2 doses (must have been after 1st birthday)
- – Hepatitis B series (if born after July 1, 1994)
- – Varicella (if born on or after April 1, 2001)
- – Tuberculin Test (only if born outside of the U.S.)

- – COVID-19 vaccine
- – Meningococcal vaccine
- – Meningococcal B vaccine
- – Hepatitis A
- – Human Papillomavirus
- – Tuberculin Test
UNCG Students have access to their personal, secure Patient Portal. Students can upload their immunization records, view their medical records schedule some appointments online, gain access to secure messaging and upload insurance cards (this process is separate from insurance verification.
