immunization requirements

North Carolina Law requires individuals attending a college or University, public, private or religious to receive certain immunizations.

Per North Carolina state law, students will be WITHDRAWN FROM THE UNIVERSITY 30 days after classes begin if immunization requirements have not been met.

The statute applies to all students except students residing off-campus and registering for any combination of:

  • Off-campus courses
  • Evening courses (classes beginning after 5 p.m.)
  • Weekend courses
  • No more than four credit hours in on-campus courses

ACCEPTABLE RECORDS OF YOUR IMMUNIZATIONS MAY BE OBTAINED FROM ANY OF THE FOLLOWING:

  • Personal Shot Records -Must be verified by a doctor’s stamp or signature or by a clinic or health department stamp.
  • Local Health Department
  • Military Records or WHO (World Health Organization) Documents -These records may not contain all of the required immunizations.
  • Previous College or University Records -Your immunization records do not transfer automatically. You must request a copy.

The immunization record must have your name, date of birth and the date the doses of vaccine were given. Each record must be verified by a healthcare provider with either their signature and clinic address or clinic stamp.

Documentation of the immunizations needs to be clear and complete. Please be certain to review the details regarding each requirement.

SHS MUST receive this information PRIOR to your arrival on campus.

Your medical history form and immunization record must be submitted through the patient portal.

After uploading records, you should receive email correspondence to your UNCG email.

INTERNATIONAL STUDENTS and/or non-US Citizens please have vaccination records translated to English prior to submission.

In addition to the required vaccines, a TB (tuberculosis) test must be submitted. All TB skin test must be completed within the United States. QuantiFERON TB testing may be accepted.

The Immunization Clinic, located on the 2nd Floor of the Anna M. Gove Student Health Center, is open year-round to administer needed immunizations, allergy shots, Depo-Provera injections, yearly Flu vaccine, Covid vaccine and Tuberculosis screening.
